Submitting Incident Reports

How to submit an incident report in the mobile app.

Submit incident reports to log any unexpected events during a shift—like falls, behaviors, or medication issues.

Incident reports must be submitted at the shift level. You can submit an incident either during an active shift or, if enabled by your agency, after a shift has ended.


Submitting an incident during an active shift

  1. Login to the app

  2. Tap View shift at the top of the Home screen

  3. Select the client you are in service with (in the Clients section)

  4. Tap Incident Reports underneath the Reports section

  5. Tap Report an Incident

  6. Select the Incident Type from the dropdown

  7. Answer each question fully. Be clear and specific for compliance and follow-up

  8. Tap Submit

The incident will be saved and associated with the current shift. After submission, administrators will receive an email notification, and the report will be available in the Dashboard for their review.


Submitting an incident for a past shift

If you forgot to submit an incident during a shift, you can submit the incident after the shift is completed (if your admin team has enabled post-shift reporting).

  1. Login to the app

  2. Scroll down to the Shifts section on the Home screen

  3. Select the Past tab

  4. Tap the past shift you want to submit the incident report

  5. Select the client in the Clients section

  6. Tap Incident Reports underneath the Reports section

  7. Tap Report an Incident

  8. Select the Incident Type from the dropdown

  9. Answer each question fully. Be clear and specific for compliance and follow-up

  10. Tap Submit

The incident will be saved and associated with the past shift. After submission, administrators will receive an email notification, and the report will be available in the Dashboard for their review.


Multiple incident reports

You can submit multiple incidents during the same shift. Simply repeat the steps above—there’s no limit to how many reports you can log. Each incident report is tied to that specific shift and client.
