Shift Notes/Client Specific Notes

Shift Notes allow agencies to collect structured documentation from DSPs or caregivers during a shift.

Where to find shift notes

  1. Open the Giv app

  2. Tap Go to Shift

  3. Look for the Notes tile on the shift screen

    • This will show all shift notes tied to the client(s) on that shift


Completing required shift notes

  • Some notes will be marked Required

  • These must be filled out before you can check out or end your shift

What happens if you skip them?

If you try to clock out without completing required shift notes:

  • You’ll see a message:

    “You must complete all required client notes before checking out clients or clocking out.”

  • The system will block your clock-out attempt


How to complete notes

  1. Tap the Notes tile

  2. Answer each question fully—these are usually written by your agency admin

  3. Tap Save

Once all required notes are completed:

  • You’ll be allowed to clock out

  • The data is stored at the shift level and tied to the client(s)


Notes may be:

  • Assigned to all clients

  • Client-specific, meaning only certain individuals will have unique note questions

  • Custom-built by your agency in the Give dashboard under the client profile


No notes showing?

If the Notes tile is empty:

  • The client doesn’t have shift notes assigned in their profile

  • Ask your admin to verify if notes should be assigned under Client Plans
