Completing Shift Notes Post-Shift

DSPs can complete notes, goals, and medication documentation after clocking out, ensuring no details are missed.

The ability to edit or update notes after a shift is completed must be enabled by your admin team on your caregiver profile.


Step 1: access the past shift

  1. Login to the app

  2. Scroll down to the Shifts section on the Home screen

  3. Select the Past tab

  4. Tap the past shift you want to update

  5. Select the client in the Clients section


Step 2: add or edit documentation

You can now log anything that was missed.

Notes

  1. Tap the Notes tile

  2. Fill out all shift notes

  3. Click Save

Goals

  1. Tap the Goals tile

  2. Select the goal worked on during the shift

  3. Enter the Time Completed on the goal

  4. Fill out each field—these are customized by your agency admin

  5. Tap Log Activity

Medications

  1. Tap the Meds tile

  2. Select the med that was administered or refused

  3. Fill out each field—these are customized by your agency admin

Incident reports

  1. Tap Incident Reports

  2. Tap Report an Incident

  3. Report incidents tied to that specific shift

  4. Select the Incident Type from the dropdown

  5. Answer each question fully. Be clear and specific for compliance and follow-up

  6. Tap Submit

Forms

  1. Tap Client Forms

  2. Select the form

  3. Fill out the information

  4. Tap Submit


Step 3: finish and return

Once your updates are submitted, you’re done! There’s no need to re-clock out—your edits are automatically tied to the shift.

This ensures documentation stays accurate for billing, compliance, and reporting.
