Managing Goals in the Client Profile

How to manage goals and track goal activity and progress for clients in the Dashboard.

The Goals tab within the Client Plans section of the client profile displays all active goals assigned to the client. Goals added to this tab are available for caregivers to view and document during their shifts, allowing agencies to track client progress and outcomes over time. If no goals are assigned, DSPs will be unable to view goals or document progress during their shifts.

Before goals are assigned to a client plan, you must first create a goal type. A goal type is a customizable questionnaire that determines the information DSPs are required to document each time they record progress toward a goal.

Step 1: Navigate to the Client Profile

To access the client's profile:

  1. Click Clients in the left menu

  2. Search for the client and select Profile

  3. Click Client Plans in the slide-out menu

  4. Select the Goals tab


Step 2: Add the Goal

To add a goal to a client's plan:

  1. Click Create Goal +

  2. Enter the Goal name

  3. Select the Goal Type from the dropdown

  4. Enter the Barrier, Strengths, and Support Strategy

  5. Click Save

Note: If no Goal Type is selected, the DSP will only be prompted to enter the Time Completed, Activity Details, and Caregiver Rating.


Editing and Archiving Goals

Goals can be Edited or Archived by clicking Details next to the goal. Once archived, you can still access the goal by clicking Filters and selecting Archived.


Tracking Goal Activity and Progress

Monitor and track goal progress in the client's profile. Goal activity can be tracked in two areas in the client's profile: the Charting and the Overview section.

Goal Charting

To view which goal activities have been logged, when, and by whom:

  1. Click Clients in the left menu

  2. Search for the client and select Profile

  3. Click Charting in the slide-out menu

  4. Click the Goals tab

You can use the search bar to search by key word, or filter the list by creator and date range.

Goal Progress Overview

To view an overview of goal progress and caregiver ratings:

  1. Click Clients in the left menu

  2. Search for the client and select Profile

  3. In the client's Dashboard, click the Goal progress & Caregiving rating tab

The Goal progress & Caregiving rating tab includes 4 charts to help monitor goal activity and statistics and caregiver ratings:

  • Goal Activity Tracker: Displays an overview of goals recorded each day within the selected week or month, along with the total number of goal activities submitted. Results are separated by goal to help monitor engagement, consistency, and overall activity trends over time.

  • General Caregiver Rating: Shows the average caregiver rating for each day goal activity was recorded, organized by goal to provide insight into daily performance feedback.

  • Percentage of shifts with activity: Displays the percentage of shifts that included at least one documented goal activity, helping track how consistently goal-related documentation is being completed.

  • Caregiver rating over time by goal: Tracks trends in caregiver ratings for each goal over time, helping identify changes in client progress, engagement, and overall performance.
