Managing Client Plans in the Client Profile

How to manage client plans in the Dashboard.

The Client Plan section within the client profile is where agencies enter and manage the majority of a client's ISP information. Any content added here is accessible to DSPs during their shifts and determines what information they can view, follow, and document while providing care.

The Client Plan includes Behavior Plans, Human Rights, Goals, Forms, and Notes. These sections help guide caregivers in delivering consistent, complaint care and ensure all required shift information is accurately recorded.

If the Client Plan is not populated, DSPs will be unable to complete or submit notes or other documentation during their shift. Because of this, it is essential to ensure all relevant information is added to the Client Plan to support accurate, complete, and compliant shift reporting.

Note: In addition to the Client Plan, caregivers can also view information entered in Basic Info and Medications. They are also able to see budget names when clocking into their shifts.


Accessing the Client Plan

To access the client plan:

  1. Click Clients in the left menu

  2. Search for the client and click Profile

  3. Click Client Plans in the slide-out menu


Step 1: Adding Behavior Plans

Add, update, and manage all behavior management strategies as found on the client’s ISP. The best practice is to copy verbatim the wording on the ISP.

To add a behavior plan:

  1. Select the Behavior Plans tab

  2. Click Create Behavior Plan +

  3. Enter the Name and Plan

  4. Click Save

Once added, DSPs will be able to view, but not edit, these behavior plans during their shift.

Click Details next to any behavior plan to Edit or Archive it. Once archived, the plan will no longer appear in the default view. To access archived plans, click Filters and choose Archived.


Step 2: Adding Human Right Restrictions

Add, update, and manage all human rights restrictions as found on the client’s ISP. This may include supervision requirements, safety equipment, behavioral interventions, or other types of restrictions. The best practice is to copy verbatim the wording on the ISP.

To add a human right restriction:

  1. Select the Human Rights tab

  2. Click Create Human Rights Restriction +

  3. Enter the Name and Plan

  4. Click Save

Once added, DSPs will be able to view, but not edit, these restrictions during their shift.

Click Details next to any entry to Edit or Archive it. Once archived, the restriction will no longer appear in the default view. To access archived restrictions, click Filters and choose Archived.


Step 3: Adding Goals

Add, update, and manage all goals as found on the client’s ISP. The best practice is to copy verbatim the wording on the ISP.

To add a goal:

  1. Select the Goals tab

  2. Click Create Goal +

  3. Enter the Goal name

  4. Select the Goal Type from the dropdown**

  5. Enter the Barrier, Strengths, and Support Strategy

  6. Click Save

Once added, DSPs will be able to view goals and log goal activity during their shift.

Step 4: Adding Forms

Add forms to the client plan for the DSP to fill out during their shift. This can include transportation forms, safety checks, ADL documentation, or other types of forms.

To add a form to the client plan:

  1. Select the Forms tab

  2. Click Add Form to Client Plan +

  3. Select the form**

  4. Click Add Forms to Client Plan

Once added, DSPs will be able to view and complete these forms during their shift.

**Forms are created in Documentation, typically during onboarding. Once created, they can be added to client plans. See Creating and Adding Forms for more details.


Step 5: Adding Shift Notes

Create and manage shift notes that caregivers can complete during or after their shift to support accurate, consistent, and compliant documentation. This helps ensure your agency captures required information in a standardized way.

To add a note:

  1. Select the Notes tab

  2. Click Add Notes +

  3. Find the desired note(s) and click the + sign next to each one

  4. (Optional) Mark the note(s) as Required

  5. Click Save

Once added, DSPs will be able to view and complete shift notes during their shift.

Outside the Client Plan

Additional sections within the client profile must be completed so caregivers can clock in and document their shifts.

Budget

Budgets must be added at the client level to allow DSPs to select a service when clocking in. See Client Profile: Budget for more information.

Medications

Medications must be added at the client level so DSPs can document medication events during their shift. See Managing Medications in the Client Profile for more information.


Charting

All documentation submitted by DSPs during their shift—including notes, goals, medications, forms, and incidents—is available in the Charting section. Documentation can be exported from this section for auditing and reporting purposes. See Client Profile: Charting for more information.
