The Notes tab within the Client Plans section of the client profile displays all active notes assigned to the client. Notes added to this tab are available for caregivers to view and document during their shifts, allowing agencies to collect required information for documentation, compliance, and reporting purposes. If no notes are assigned, DSPs will be unable to submit shift notes while documenting their shifts.
Notes are created and managed in Feature Settings. When creating a new note, you can customize how it is used throughout your organization. For example, notes can be marked as required or automatically assigned to all current and/or future clients.

Step 1: Navigate to the Client Profile
To access the client's profile:
Click Clients in the left menu
Search for the client and select Profile
Click Client Plans in the slide-out menu
Select the Notes tab
Step 2: Add the Note
To add a note to a client's plan:
Click Add Notes +
Find the desired note(s) and click the + sign next to each one
(Optional) Mark the note(s) as Required
Click Save
All notes assigned to the client are displayed on this page. When a DSP documents their shift in the app, they will see all assigned notes and must complete any notes marked as required before they can clock out.
If the note has not yet been created, click Create New Note to create it in Feature Settings. Once created, the note can be assigned to the client's plan.
Editing Notes
Notes are edited in Feature Settings. Any changes made to a note will apply to all client plans where that note is assigned.
Click the dropdown arrow in the top left corner
Click Feature Settings
Select the Shift settings card
Scroll down to the Shift Notes section
Click Manage
Click the 3 dots next to the note you want to edit
Select Edit
Make the changes
Click Save Changes
Archiving Notes in the Client Plan
To archive a note in a client's plan:
In the Notes tab, click Details next to the note
Click Archive
Archiving a note in the client's plan removes it from the client profile, so caregivers will no longer see it in their app. However, the note will remain available in Feature Settings and can still be assigned to other clients.
Tracking Shift Note Submissions
Monitor note submissions directly from the client's profile. All submitted notes can be viewed in the Charting section.
To view which notes have been submitted, when, and by whom:
Click Clients in the left menu
Search for the client and select Profile
Click Charting in the slide-out menu
Click the Notes tab
You can use the search bar to search by key word, or filter the list by creator and date range.
