Overview: Forms

Overview of the Forms section in the mobile app.

The Forms section allows caregivers to complete required forms outside of a shift. This functionality helps streamline documentation and ensures all necessary information is captured efficiently.


Accessing Forms

  1. Login to the app

  2. Tap Forms at the bottom of the screen

The Forms section has 3 tabs:

  • Fill Out Form: Displays all forms that can be completed outside of a shift. These forms were all created by your admin team. By default, you will land on this tab when you tap Forms.

  • Drafts: Displays all forms you have saved as a draft.

  • Submitted: Displays all forms you have submitted in the mobile app.

Note: Transportation forms will not be displayed in this section. Transportation forms are only available to submit through the shift itself.


Fill Out Form

To fill out a form:

  1. In the Fill Out Forms tab, select the form you want to fill out

  2. Enter all required information

  3. Tap Submit

The form will be visible in the Submitted tab. Your admin team will be able to view the form in the Dashboard as well.


Drafts

If you need to navigate away from a form before completing it, tap Save as Draft at the top of the screen. Your progress will be saved and located in the Drafts tab.

To reopen a draft, click the Drafts tab, then select the draft you want to open.


Submitted

All forms submitted through the mobile app—except transportation forms—will appear in the Submitted tab, whether they were completed in the Forms section or completed directly within a shift. Select any form to view the full submission.
