Overview: Forms

Overview of the forms section in the mobile app.

The Forms section allows caregivers to complete required forms outside of a shift. This functionality helps streamline documentation and ensures all necessary information is captured efficiently.


Accessing Forms

  1. Login to the app

  2. Tap Forms at the bottom of the screen

The Forms section has 3 tabs: Fill Out Form, Drafts, and Submitted.

By default, you will land on the Fill Out Forms tab, which displays forms that can be completed outside of a shift. These forms were all created and customized by your admin team in the Dashboard.


Completing Forms

To fill out a form:

  1. Select the form you want to fill out

  2. Enter all required information

  3. Tap Submit

If you need to navigate away before completing the form, tap Save as Draft. Your progress will be saved and located in the Drafts tab.


Submitted Forms

All forms that you have submitted through the Forms section or submitted directly in the shift will appear in the Submitted tab. Tap on any form to view its details.
