Submitting Transportation Forms

How to access and submit transportation forms, as a DSP, in the mobile app.

Transportation forms ensure accurate tracking and billing of travel related to client services. All transportation in Giv—whether trip-based or mileage-based—is recorded using designated Transportation Forms. These forms are created by your agency’s admin team and completed by caregivers during a shift.


Step 1: access transportation forms

Transportation forms are best accessed while you are on an active shift.

  1. Login to the app

  2. Tap View shift at the top of the Home screen

  3. Scroll down to the Transportation section and tap Transportation Forms

Only transportation forms that have been set up and assigned by your agency will appear here.


Step 2: select and complete the transportation form

  1. Tap the transportation form you want to fill out

  2. Select the client in the Clients Transported dropdown

  3. Enter the Odometer Start and Odometer End

    • Some states require both the starting and ending odometer readings. If your state does not require this, enter the milage difference instead.

  4. Fill out any other details

  5. Click Submit

Complete the transportation details based on the type of form:

  • Trip-Based: the form counts as a single transportation unit (e.g., 1 form = 1 trip-based unit).

  • Mileage-Based: the mileage difference matters (e.g., if you traveled 10 miles put the start odometer as 1 and the end odometer as 11).

If you are required to track transportation to and from a client separately (such as pickup and drop-off), be sure to submit two transportation forms: one for retrieving the client and one for dropping off the client.

Important: The Odometer Start cannot be 0.
