Each tab in the client profile plays a specific role in enabling documentation, care tracking, and reporting.


Basic info

  • What to enter: Name, address, date of birth, Medicaid ID, and any key identifiers.

  • Why it matters: This is the first information DSPs see when working with a client. Keep it clear and concise.


Health

  • Add relevant medical details for DSP visibility.

  • If a field is not relevant to your agency or client, it's okay to leave it blank.


About & Support

  • About: Help staff get to know the individual—interests, routines, communication preferences.

  • Support: Provide guidance on how to best support them (e.g., preferred calming techniques, behavior triggers).


Client plans

  • Add Behavior Plans and describe how to follow them.

  • Clearly document any Human Rights Restrictions (e.g., water consumption limits) to inform DSPs of safety considerations and compliance.


Goals

  • Important: If goals are not entered here, DSPs cannot log progress on them during shifts.

  • Add each goal that staff should track, such as daily living skills, communication efforts, etc.


Forms

  • Attach custom forms to clients for shift documentation.

  • Forms are fully customizable (e.g., incident forms, toileting logs, transportation forms).

  • DSPs can complete these during their shifts, and the data is tied to that client.


Notes

  • Used for shift notes or client-specific admin notes.

  • These are separate from goals and more focused on internal documentation or compliance tracking.

  • Can be applied to all clients or customized per client.

📝 If notes aren’t assigned to a client, the DSP won’t see any options to log during the shift.


Medication

  • Log all current medications, dosages, and administration instructions.

  • If nothing is entered, the DSP won’t be able to view or record med administrations or refusals during the shift.

  • You can set reminder times—these push notifications alert the DSP when logged into the app.


Documents & Files

  • Upload additional documentation as needed (e.g., care plans, assessments, consent forms).

  • These stay tied to the client’s record for future reference.


Keep profiles updated

After onboarding, it's up to your agency to regularly review and update client records—especially medications, goals, and forms—to ensure accurate care and compliance.
