Understanding Documentation in Giv

Learn the different types of documentation available in Giv, including Shift Notes, Goals, Medications, Incident Reports, and Custom Forms.

Documentation is one of the most important parts of using Giv. During a shift, you may be asked to complete one or more types of documentation depending on your agency's requirements.

Each documentation type serves a different purpose, helping your agency maintain accurate records, meet compliance requirements, and track the support provided to individuals.

Shift Notes

Shift Notes are used to document the services and support provided during a shift.

Your agency may require Shift Notes before you can clock out, ensuring important information is captured every time services are delivered.

Common information recorded in Shift Notes includes:

  • Support that was provided

  • Activities completed

  • Concerns or observations

  • The individual's response to services

Because Shift Notes are designed for recurring documentation, they're often one of the most frequently used features in Giv.

Goals

Goals help track an individual's progress toward specific outcomes or objectives.

Your agency can create different Goal Types, each with its own questions and documentation requirements. For example, documenting progress on a hygiene goal may look different from documenting a community integration goal.

When recording goal progress, staff may be asked to document:

  • The activity completed

  • Time spent working on the goal

  • Progress or outcome

  • Ratings or other required information

This allows agencies to consistently measure progress over time.

Medications

The Medications feature is used to document medication administration and other medication-related activities.

Depending on the medication, staff may be asked to record different information. For example, one medication may only require the administration time, while another may also require blood glucose readings or dosage information. Medication Types allow agencies to customize the documentation required for different medications.

Incident Reports

Incident Reports are used whenever an event occurs that requires additional documentation.

Your agency may have different Incident Types for situations such as:

  • Medical emergencies

  • Behavioral incidents

  • Medication errors

  • Missing person incidents

  • Staff-related incidents

Each Incident Type can require different questions to ensure the appropriate information is collected before the report is submitted.

Custom Forms

Sometimes information doesn't fit neatly into Shift Notes, Goals, Medications, or Incident Reports. That's where Custom Forms come in.

Agencies use Custom Forms for a wide variety of purposes, including:

  • Transportation logs

  • Staff checklists

  • Client forms

  • Administrative documentation

  • Forms completed outside of a shift

Custom Forms can be configured for use in the Dashboard, the mobile app, or both, depending on your agency's needs.

The information you document doesn't just create a record of the services you provided, it also supports many other parts of the platform.

Documentation can be used for:

  • Billing

  • Timesheets

  • Authorization tracking

  • Compliance

  • Reporting and analytics
