The Charting section of the client profile contains all submitted notes, goals, medication records, forms, and incident reports for the client. Most documentation is entered by caregivers during or after their shifts, but admins can also create and manage charting records directly from the Dashboard.
Viewing Charting Records
To view a client's charting records:
Click Clients in the left menu
Search for the client and select Profile
Click Charting in the slide-out menu
Click the Notes, Goals, Medications, Forms, or Incidents tab to view that documentation
Use the search bar to find records by keyword, or filter the list by creator and date range to narrow your results.
Adding Charting Records
Documentation can be added directly from the Charting section of a client's profile. However, we recommend adding documentation through the shift whenever possible, as this automatically associates the record with the shift.
Adding Documentation from the Client Profile
Open the client's Charting section
Select the tab where you want to add documentation
Click Add or Log in the top-right corner
Select the note, goal, medication, form, or incident report you want to submit
Enter the required information
Click Save
The documentation will appear in the client's charting history and remain associated with the client. However, it will not be linked to a specific shift.
Adding Documentation from a Shift
Open the shift from either the Calendar or Scheduling section of the Dashboard.
Click the Charting tile
Select the tab where you want to add documentation
Click + Add or + Log in the top-right corner
Enter the required information
Click Save
The documentation will be linked to the shift and will also appear in the client's charting history.
Editing Charting Records
There may be times when a caregiver needs to update documentation after a shift has been completed. Depending on your organization's settings, caregivers may be able to add or edit notes for past shifts. Admins can also edit charting records from the Dashboard.
Open the client's Charting section
Select the tab where you want to edit documentation
Locate the note, goal, medication, form, or incident report
Hover over the Report column for that edit and click Edit when it appears
Make the necessary changes
Click Save changes
The updated information will appear in both the client's Charting section and the Charting section of the associated shift.
Exporting Charting Records
You can export charting records to a CSV file for reporting and record-keeping purposes:
Open the client's Charting section and select the tab ycontaining the records you want to export
Check the boxes next to the records you want to include. To select all visible records, check the box in the top-left corner of the table.
Click the Export icon in the top-right corner
(Optional) Click Edit Headers to customize the column names included in the export
Click Export Records
The selected records will be downloaded to your computer as a .csv file.
Note: By default, only the first 25 records are loaded in the table. If you need to export additional records, scroll to the bottom of the list to load all entries before exporting. The export will include all loaded records and display the total number of records being exported.
