Desktop Charting: Clocking In and Completing Shift Documentation

How to complete shift documentation as a DSP using desktop charting.

Caregivers can complete shift documentation through desktop charting or the Giv mobile app. Desktop charting allows caregivers to review client information and enter documentation using a computer.

By default, caregivers must be clocked in and actively running a service to complete shift-based charting. Depending on agency settings and caregiver permissions, additional access may be available to review client records, document outside an active service, or complete documentation after a shift has ended.

Step 1: Log In to Desktop Charting

Every caregiver can access desktop charting:

  1. Open a web browser and go to dashboard.giv.plus

  2. Enter the phone number and password you use to log in to the Giv mobile app

  3. Enter the confirmation code when prompted

After logging in, you will land on the My Shifts page.

My Shifts

At the top of the page, you will see your next scheduled shift. Select Go to shift to open it.

Below your next scheduled shift, you can view your upcoming shifts. By default, the list displays shifts scheduled over the next two weeks. You can adjust the date range to view past or future shifts and filter by client or status to find a specific shift.

Viewing Past Shifts

If your agency has enabled post-shift reports, you may be able to open completed shifts to review previously submitted documentation and add or edit documentation, depending on your permissions.

To open a past shift, adjust the date range and select the shift from the list.

If post-shift reports is not enabled, you will see the following message: "You don't currently have access to view post-shift reports. Please contact your admin for assistance." Contact your agency administrator if you need access to past shift documentation.

Clients

If your administrator has enabled Advanced Charting Access on your caregiver profile, a Clients section will appear in the left-side navigation menu. This section displays the clients you have been granted access to.

Select a client to open their profile and view available information:

  • Basic Info: General information, health details, personal information, and support details

  • Budget: Service codes and their effective dates. Financial information is not displayed here

  • Medications: Medications listed on the client's profile

  • Charting: Previously submitted notes, goals, medications, forms, and incidents. To add documentation, select Add in the top-right corner of the appropriate tab

  • Documents: Documents added to the client's profile that your agency has made available to caregivers

  • Contacts: Contacts added to the client's profile

Important: Documentation added directly through a client's profile is not tied to a specific shift. It can be viewed by the admin team in the client's records.

With Advanced Charting Access enabled, you can also access client records and complete charting for clients you are not actively providing a service to. This can be useful for managers and other staff who need to review or update documentation outside an active service.

Without this access, you will see the following message when selecting a client you are not in service with: "You must either be in service or have been in service with [client] during this shift in order to view client records." Contact your administrator if you need access to records outside of a shift.

Step 2: Clock In to Your Shift

There are two ways to begin a shift before completing charting on your computer. Choose the method that works best for you.

Important: You must use the Giv mobile app to complete the clock-in process and start the service when your agency requires you to be clocked in and actively running a service before charting.

Option 1: Clock In Through the Giv Mobile App First

Use this option if you want to start your shift on your phone before opening desktop charting.

  1. Open the Giv mobile app and log in

  2. Clock in to your scheduled shift and start the service

  3. On your computer, go to dashboard.giv.plus and login

Your active shift should appear at the top of the My Shifts page as an in-progress shift.

You can now continue completing documentation on your computer.

Option 2: Start on Desktop and Finish in the Giv Mobile App

Use this option if you want to begin the clock-in process on your computer.

  1. On your computer, go to dashboard.giv.plus and login

  2. Open your scheduled shift. If it is your next scheduled shift, you can select Go to shift

  3. Select Shift Actions and choose Clock in

  4. A QR code will appear on your computer screen. Open your phone and scan the QR code

  5. Complete the clock-in process and start the service for the client in the mobile app

  6. Return to desktop charting and refresh the page

Your shift should now appear as In Progress, allowing you to continue charting on your computer.

Step 3: Completing Shift Documentation

Once your shift is in progress:

  1. Go to the Shifts page

  2. Select Go to shift to open your active shift, if it is not already open

  3. Locate the Clients section below the shift information

  4. Select the client you want to document

The Clients section displays the clients assigned to the shift and the service currently in progress for each client, when applicable.

By default, you can access client records through a shift when you are in service with that client or have been in service with them during the shift. If Advanced Charting Access is enabled, you may also be able to access client records without an active service.

Entering and Reviewing Documentation

After selecting a client, you can add new documentation or review documentation already submitted during the shift.

Available documentation includes: notes, goals, medications, forms, and incidents.

The charting screen is divided into two sections:

  • Left side: Select the appropriate documentation tab, enter the required information, and select Save to submit your documentation

  • Right side: Review the history of documentation completed for the shift

Make sure you complete all required documentation for the client and shift before clocking out.

For instructions on completing documentation through the mobile app, see Documentation & Reporting.

Step 4: Clocking Out of Your Shift

Clocking out must be completed through the Giv mobile app. You can initiate the process from your phone or from desktop charting.

Option 1: Clock Out Through the Giv Mobile App

  1. Open the Giv mobile app

  2. Locate and open your active shift

  3. Follow the prompts to clock out

Option 2: Start on Desktop and Finish in the Giv Mobile App

  1. On your computer, open your active shift in desktop charting

  2. Select Shift Actions and choose Clock Out

  3. A QR code will appear on your computer screen. Open your phone and scan the QR code

  4. Follow the prompts in the app to complete clock-out

If your agency requires specific documentation before clocking out, make sure you complete those requirements first.

Complete Documentation For a Past Shift

If your agency has enabled post-shift charting, you may be able to review, add, or edit documentation for a completed shift, depending on your access permissions.

  1. On your computer, go to dashboard.giv.plus and login

  2. On the My Shifts page, adjust the date range to locate the past shift

  3. Open the shift

  4. Select the client you need to document

  5. Choose the appropriate tab: Notes, Goals, Medications, Forms, or Incidents

  6. Enter or update the required information

  7. Click Save.

The documentation will be saved to the selected shift.

If you cannot open a past shift or add documentation, contact your agency administrator to confirm that post-shift reports are enabled and that you have the necessary permissions.

For instructions on completing documentation for past shifts in the Giv app, see Completing Shift Documentation Post-Shift.
