An incident type is a customizable questionnaire or form template that determines which questions a DSP must answer when submitting an incident report.
You can create different incident types for specific situations to ensure that all required information is collected for each incident. For example, separate incident types can be created for medical emergencies, missing persons, or staff incidents.
Incident types help ensure documentation aligns with state requirements, compliance standards, and internal workflows.
Note: When submitting an incident report, caregivers must first select the appropriate incident type. They are required to complete all mandatory fields before the report can be submitted.
Step 1: Create and Name the Type
Click the dropdown arrow in the top left corner
Click Feature Settings
Select the Incidents card
Click + Add new type
Enter the Incident Type name
Step 2: Customize the Fields
After naming the incident type, customize the fields using one of the following options:
Option 1: Copy the fields from an existing incident type
Click the Select incident type dropdown
Select the incident type you want to copy fields from
Make any necessary edits or add additional fields
Option 2: Create the form from scratch
Click + Add Field
Enter the Field Name
Click the Field Type dropdown and select the desired field type. Available options include:
Date and Time
Long Text
Multiple Choice
Number
Photos/Video
Short Text
Signature (Digital or Drawn)
Single Choice
Toggle
(Optional) Mark the field as Required
Repeat these steps until all fields are created
Step 3: Save the Incident Type
Once the type is built as desired, click Save Changes.
The incident type will now be available for caregivers to select when submitting an incident report.
Editing and Archiving Incident Types
Incident types can be Edited or Archived by clicking Manage next to the type.
