The Client Plan contains information that DSPs can view and documentation that DSPs can complete during their shift. It includes Behavior Plans, Human Rights, Goals, Forms, and Notes. If the client plan is empty, DSPs will be unable to add notes and log progress during their shift.
Accessing the client plan
Click Clients in the left menu
Search for the client and click Profile
Click Client Plans in the slide-out menu
Adding behavior plans
Add, update, and manage all behavior management strategies as found on the client’s ISP.
Best practice is to copy verbatim the wording on the ISP. Once added, DSPs will be able to view, but not edit, these behavior plans during their shift.
To add a behavior plan:
Click on the Behavior Plans tab
Click Create Behavior Plan +
Enter the Name and Plan
Click Save
Click Details next to any behavior plan to Edit or Archive it. Once archived, the plan will no longer appear in the default view. To access archived plans, click Filters and choose Archived.
Adding human right restrictions
Add, update, and manage all human rights restrictions as found on the client’s ISP. This may include supervision requirements, safety equipment, behavioral interventions, or other types of restrictions.
Best practice is to copy verbatim the wording on the ISP. DSPs will be able to view, but not edit, these restrictions during their shift.
To add a human right restriction:
Click on the Human Rights tab
Click Create Human Rights Restriction +
Enter the Name and Plan
Click Save
Click Details next to any entry to Edit or Archive it. Once archived, the restriction will no longer appear in the default view. To access archived restrictions, click Filters and choose Archived.
Adding goals
Add, update, and manage all goals as found on the client’s ISP.
Best practice is to copy verbatim the wording on the ISP. DSPs will be able to log and manage goal progress during their shift.
To add a goal to the Client Plan:
Go back to Client Plans in the client’s profile
Click on the Goals tab in the Client Plans
Click Create Goal +
Enter the Goal name
Select the Goal Type from the dropdown**
Enter the Barrier, Strengths, and Support Strategy
Click Save
Click Details next to any goal to Edit or Archive it. Once archived, the goal will no longer appear in the default view. To access archived goals, click Filters and choose Archived.
**Goal types—the questions DSPs complete when logging a goal—are created in Feature Settings, typically during onboarding. Once created, goal types can be used across all client plans. See Feature Settings: Goals for more details.
Adding forms
Add forms to the client plan for the DSP to fill out during their shift. This can include transportation forms, safety checks, ADL documentation, or other types of forms. Once added, DSPs will be able to complete these forms during their shift.
To add a form to the client plan:
Click the Forms tab
Click Add Form to Client Plan +
Select the form**
Click Add Forms to Client Plan
**Forms are created in Documentation, typically during onboarding. Once created, they can be added to client plans. See Creating and Adding Forms for more details.
Adding shift notes
Add notes to the Client Plan for DSPs to complete during their shift. Shift notes serve as daily documentation and can be required for DSPs to complete before clocking out. They are included in data exports to support reporting, auditing, and compliance.
To add a shift note to the client plan:
Click on the Notes tab
Click Add Notes +
Add notes using one of the following options:
Option 1: add an existing note
Click the + sign next to the note you want to add
Toggle on Required if desired
Click Save
Option 2: create a new note (if missing from the list)
Click Create New Note
Click + New Note
Fill out the required information
Click Create Note
Return to the client’s profile and add the note to the Client Plan
Click Details next to any note to Edit or Archive it. The only edit available is to turn on/off the Required setting. If you need to make edits to the actual note, that will be done in Feature Settings. See Feature Settings: Shift Notes for more details.
Note: Shift notes are typically created during onboarding. Once created, they can be added to or applied across all client plans.
Outside the client plan
Additional sections within the client profile must be completed so caregivers can clock in and document their shifts.
Budget
Budgets must be added at the client level to allow DSPs to select a service when clocking in. See Client Profile: Budget for more information.
Medications
Medications must be added at the client level so DSPs can document medication administration during their shift. See Client Profile: Medications for more information.
Charting
All documentation submitted by DSPs during their shift—including notes, goals, medications, forms, and incidents—is available in the Charting section. Documentation can be exported from this section for auditing and reporting purposes. See Client Profile: Charting for more information.
