Customizing Client Plans

How to customize the client plan for DSPs to fill out during a shift.

The Client Plan contains information that DSPs can view and complete during their shift. It includes Behavior Plans, Human Rights, Goals, Forms, and Notes. If the client plan is empty, DSPs will be unable to add notes and log progress during their shift.


Accessing the client plan

  1. Click Clients in the left menu

  2. Search for the client and click Profile

  3. Click Client Plans in the slide-out menu


Add behavior plans

Add and manage any behavior management strategies as found on the client’s ISP.

Best practice is to copy verbatim the wording on the ISP. Once added, DSPs will be able to view, but not edit, these behavior plans during their shift.

To add a behavior plan:

  1. Click on the Behavior Plans tab

  2. Click Create Behavior Plan +

  3. Enter the Name and Plan

  4. Click Save


Add human right restrictions

Add and manage any human rights restrictions as found on the client’s ISP. This may include supervision requirements, safety equipment, behavioral interventions, or other types of restrictions.

Best practice is to copy verbatim the wording on the ISP. DSPs will be able to view, but not edit, these restrictions during their shift.

To add a human right restriction:

  1. Click on the Human Rights tab

  2. Click Create Human Rights Restriction +

  3. Enter the Name and Plan

  4. Click Save


Add goals

Add and manage all goals as found on the client’s ISP.

Best practice is to copy verbatim the wording on the ISP. DSPs will be able to log and manage goal progress during their shift.

To add a goal to the Client Plan:

  1. Go back to Client Plans in the client’s profile

  2. Click on the Goals tab in the Client Plans

  3. Click Create Goal +

  4. Enter the Goal name

  5. Select the Goal Type from the dropdown**

  6. Enter the Barrier, Strengths, and Support Strategy

  7. Click Save

Once created, goals can be edited or archived.

**Goal types—the questions DSPs complete when logging a goal—are created in Feature Settings, typically during onboarding. Once created, goal types can be used across all client plans. See Account Settings: Goals for more details.


Add forms

Add forms to the client plan for the DSP to fill out during their shift. This can include transportation forms, safety checks, ADL documentation, or other types of forms. Once added, DSPs will be able to complete these forms during their shift.

To add a form to the client plan:

  1. Click the Forms tab

  2. Click Add Form to Client Plan +

  3. Select the form**

  4. Click Add Forms to Client Plan

**Forms are created in Documentation, typically during onboarding. Once created, they can be added to client plans. See Creating and Adding Forms for more details.


Add shift notes

Add notes to the Client Plan for DSPs to complete during their shift. Shift notes serve as daily documentation and can be required for DSPs to complete before clocking out. They are included in data exports to support reporting, auditing, and compliance.

To add a shift note to the client plan:

  1. Click on the Notes tab

  2. Click Add Notes +

Add notes using one of the following options:

Option 1: add an existing note

  1. Click the + sign next to the note you want to add

  2. Toggle on Required if desired

  3. Click Save

Option 2: create a new note (if missing from the list)

  1. Click Create New Note

  2. Click + New Note

  3. Fill out the required information

  4. Click Create Note

  5. Return to the client’s profile and add the note to the Client Plan

Note: Shift notes are created in Feature Settings, typically during onboarding. Once created, they can be added to or applied across all client plans. See Account Settings: Shift Notes for more details.


Medications

Medications are managed in their own section outside of Client Plans. They must be added at the client level for DSPs to log medication administration during their shift. See Client Profile Overview: Medications for more information.


Charting

All documentation submitted by DSPs during their shift—including notes, goals, medications, forms, and incidents—is available in the Charting section. Documentation can be exported from this section for auditing and reporting purposes. See Client Profile Overview: Charting for more information.
